I don't think the Marianas have malaria - I know they don't now, but if malaria was present there in WW2, I've never heard such.

 

And apparently there was malaria in Okinawa.  But only in the north, while the fighting was all in the south, and the outbreak in the north was after the battle, and among civilians.  So if your grandfather got malaria in Okinawa, he had to have been extremely unlucky (like, "hit by lightning" unlucky.)
